I know that from my own personal experience, I found that the abilities of the person in 'the official position' were severely limited, and know quite a few people who are much more skilled than I am have said they aren't a Chiurgeon because of it. Because if they want to help someone, and they see it happening, they can unofficially use all the skills they have but can't officially.
