Two weeks ago, ordered a passport. Paid expedited, as to get it faster. Should have gotten it today.
No, got a letter from the Department of State.
Okay, quick note. The last name most people know me by is *not* the last name I was born under. My drivers licence, ID cards. bank statements, credit cards, phone bills, car registration, student loans, car insurance, voter registration, etc., all use the last name most people know me by, hyphenated with my old last name. And Little Bit and Littlest Bit's birth certificates both have the name I use the most on it.
And, when I registered for the passport, I used my old last name on it, with a "as known by" addition in the appropriate portion of the form, along with my drivers licence and birth certificate.
This does not seem to be good enough for them. I need "three public records", (school, military, insurance, medical, etc.), showing the current use of" my known as name. For *one* of those records, I can substitute notarized affifavits (DS-60) from two persons who have known me for at least a year.
However, this delay means it's unlikely I'll have my passport before my vacation starts.
So, what I get to do tomorrow is:
Call the state department, find out exactly what public records they find acceptable, and can they accept faxed copies. Also find out if they accept a DMV printout, since that should show exactly when I started offically using my current last name.
Order a second copy of my birth certificate, send via priority mail.
I've been told I can cross into Canada using only my drivers licence and birth certificate. I hope this is indeed the case. I'd hate to get as far as Seattle, and have my friends in Vancouver have to cross to my side of the border to visit.
